The Challenge
A patient first presented to our office at the age of 34. He had been a long-time RGP lens wearer due to keratoconus. His first visit to our practice was in 2015, when he complained of recurrent eye infections, excessive photophobia and general contact lens intolerance. After treating his corneal erosions, we discussed the benefits of scleral contact lenses and fitted him for Europa lenses.
The Solution
The initial fitting of a keratoconic patient wearing RGP lenses for many years is often difficult. The uncertainty of corneal reshaping following the discontinuation of the RGP lenses makes initial lens selection more complicated. Base curve changes will be required as the cornea reshapes. It’s important to educate and prepare the patient in advance, as multiple visits are necessary for a successful first fitting of a scleral lens.

The Results
The patient has been successfully wearing his scleral lenses for the past six years, with minor changes to the base curve and power, adjusting for his progressive keratoconus. His vision remains OD: 20/25 and OS: 20/40 with a central stromal scar in his left eye.
