Dr. Frankel shares how the sMap3D™ has helped her customize lenses for patients with irregular scleras or corneas, resulting in a successful fit for even the most challenging cases.

The Visionary Optics team spoke with Stephanie Frankel, OD, from Bascom Palmer Eye Institute in Miami, Florida about how the sMap3D™ has opened up new lens-fitting possibilities and improved the lives of her patients. Many of Bascom Palmer’s patients have irregular scleras, and without image-guided technology, they would be extremely difficult to fit successfully with conventional lenses.
“The sMap3D provides an excellent fit for irregular scleras, uniform clearance over irregular corneas, and great optics for optimal visual rehabilitation,” said Dr. Frankel. “It offers options to customize the lenses with very little limitation. Being able to use the sMap3D on my patients to evaluate scleral toricity and subsequently choose an optimal scleral lens option has been incredible for my practice and the overall success of our scleral lens program. I don’t know how we would have been able to have the success stories that we have without this technology.”

The Challenge
A 39-year-old patient was referred to the clinic with a history of bilateral HSV keratitis that left significant corneal scarring, neurotrophic keratopathy, and limbal stem cell deficiency. He had post penetrating keratoplasty in his right eye, as well as a glaucoma drainage implant.
Due to his stem cell deficiency, he was in bandage contact lenses constantly to assist in preventing non-healing corneal epithelial defects. He developed secondary scarring from his recurring corneal epithelial defects. He had to undergo two rounds of Oxervate and still had 20/200 vision (OU) and recurring epithelial defects.
The Solution
After being fit with Latitude lenses in both eyes, the patient has been without any corneal epithelial defects in his right eye, and we were able to give 20/25 visual rehabilitation to him in his left eye. Now that his corneas have remained stable, he will soon be able to undergo cataract extraction in his right eye, for a mature cataract, to further improve his vision.
The Results
The patient is overwhelmed with his result and has expressed that these lenses have “saved him.” He wrote this shortly after receiving his new lenses:
