A 56-year-old male with severe keratoconus, worse in the left eye, was refit from hybrid lenses into Europa Tangent scleral lenses. Anterior segment OCT played a crucial role in optimizing the fit. The right eye achieved an even corneal-lens relationship, with proper limbal clearance and edge alignment, requiring only one lens remake. The left eye, with advanced ectasia, initially showed 700 microns of central clearance and 258 microns of apical clearance. Customization reduced apical clearance to ~100 microns post-settling and achieved 327 microns of central clearance in the final design. OCT proved invaluable for documenting and refining the fitting process, ensuring comfort and corneal health.
Dr. Emmy Tian is a native of Reno, Nevada. She received her Bachelor of Science in Chemical Biology and Doctor of Optometry degrees from UC Berkeley. Following optometry school, Dr. Tian completed a residency in Cornea and Contact Lenses at the Southern California College of Optometry (SCCO) at Marshall B. Ketchum University.
